Transition for Pupils with SEND
Effective transition at all stages is essential to ensure that children feel safe within the learning environment. At Oxclose Primary Academy, we have systems in place to ensure that transition is as smooth as possible for all of our pupils.
Transition Into Reception
Transition from Nurseries into Reception is a big step for children and their parents. We aim to support our families through this important stage in a number of ways. Our Early Years Lead works closely with staff from a range of nurseries, to ensure that the transition process is as smooth as possible.
Prior to starting Reception, parents are invited to attend an induction meeting. During this meeting there is an opportunity to meet all Early Years staff, the Head Teacher and SENDCo.
Transition Between Classes
At Oxclose Primary we plan transition carefully for all pupils. Pupils will access transition sessions, where they meet the new staff they will be working with. Some pupils may require additional transition arrangements, this can include spending additional time in their new classroom, visiting their new classroom with their current member of support staff or beginning transition arrangements earlier in the term.
Some pupils will have access to a transition booklet, this includes photographs of their new classroom, key areas of the school and staff working in their new class. Pupils will take this home before the end of the summer term to share with their parents and use during the summer holidays.
In Summer Term, all pupils who are on the SEND Register, will have a Pupil Passport written by the staff currently supporting them. This will be shared with their new class teacher to ensure they have relevant and up to date information about the pupils’ needs to support with transition.
The support provided can be tailored to the needs of individual pupils and can be discussed with parents/carers and pupil.
Transition Into Key Stage Three
Once secondary school places have been allocated, the Year 6 Class Teacher and SENDCo liaise with staff from the secondary schools to ensure they have all the information needed to support a smooth transition. The SENDCo will liaise with the receiving SENDCo to discuss pupil need and, at times, the receiving school will arrange additional transition sessions. The support provided can be tailored to the needs of individual pupils and can be discussed with parents and carers and pupil.
Joining Our School at Other Times
If your child joins our school during a Key Stage or mid-way through the academic year, systems are in place to support their transition from their previous school. If your child has SEND, our SENDCo will liaise with the SENDCo from your child’s previous school to ensure that we have the required information to enable us to support your child fully. It would be beneficial if you could discuss your child’s needs with us and share any information you have when you visit.
Leaving Our School
If your child leaves Oxclose Primary Academy, the SENDCo will ensure that all SEND information is passed onto the next school to ensure that they are fully aware of any additional needs your child may have.
